Student Waivers & Emergency Contact Information
Every student on each team must complete and submit a signed waiver form and submit emergency contact information before their team will be allowed to participate in the tournament on campus. Please complete the forms and submit them by Dec 6.
To sign the waiver, click the button below, select File > Download, and send the file to your students and their guardians for electronic signature.
Once you have received the signed waivers from your students and their guardians, click the button below to upload the completed forms and one additional file listing all the students you are bringing and their emergency contact.
